I consent to have my child participate in Richmond Heights Memorial Library (RHML) youth camp Mad Science Rocketry Camp. I recognize that RHML Camp Programs involve certain risks and dangers and that accidents may occur despite all reasonable care and prevention. In consideration of the services rendered to me and my child by RHML, I (including my heirs, personal representatives and assignees) hereby RELEASE and hold harmless RHML, its members, officers, representatives, agents and employees from any and all actions, claims, demands, obligations, liabilities, and damages which I or my child may have against the Released Parties for all injuries, known or unknown, and damages arising, if any, out of participation in RHML Camp activities and programs. I further agree, individually and on behalf of my child, to indemnify and hold harmless the RHML and its respective members, officers, employees, agents and sponsors of the RHML camp, from any and all claims, actions, damages, expenses, costs (including attorneys’ fees), lawsuits or judgments of any nature which result from any and all acts of my child in relation to the RHML Camp program, including their own negligence. I authorize the RHML to consent for any necessary emergency or urgent medical treatment required by my child while under RHML supervision. This permission includes, but is not limited to, the administration of first aid and the use of an ambulance. I agree to be financially responsible for any charges incurred for the necessary treatment for my child.
I do hereby grant permission to Mad Science to photograph my child named above, enrolled in a Mad Science Summer Camp. If photographed, my child’s photo may possibly be used in promotional media, including printed brochures and Mad Science’s web page. It is Mad Science’s policy to never disclose a child’s identity without the expressed written authorization of the child’s parents or guardians. Accordingly, my child will not be identified if his or her photograph is used for promotional purposes.
